We cannot help with a complaint about a facility, advise whether a facility is right for someone, or tell you whether a resident is safe. For a concern about a resident’s care, every state has a long-term care ombudsman whose job is exactly that, and Medicare.gov’s Care Compare lists the state survey agency for each facility; 1-800-MEDICARE can route you to both. We can tell you what is in the record and where it came from.
