What this site is

We republish the nursing home data the Centers for Medicare & Medicaid Services publishes in its Provider Data Catalog: ratings, health inspection citations, penalties and ownership. We are not affiliated with CMS, Medicare, any state survey agency or any facility listed, and nothing here is an official record.

What a rating and a citation are

Every star rating on this site is CMS’s own rating, shown with the footnote CMS attaches where a rating is missing. Every citation is a finding by a state surveyor on the date shown, in CMS’s own words, with the severity CMS assigned and whether the facility reported correcting it. We do not rank facilities, we do not estimate anything the file does not state, and this site is not a recommendation of any facility, nor one against it.

Information, not advice

Nothing on this site is advice: it is not medical advice, not legal advice, and not a substitute for visiting a facility, speaking to its staff and residents, and speaking to your state’s long-term care ombudsman. Whether a facility is right for a person is a question the record cannot answer.

Accuracy

We show what the source files contain, including their errors and their lag: CMS refreshes the files monthly, a citation can take months to appear, and a facility’s record can change the day after we read it. We make no warranty of accuracy, completeness or fitness for a purpose, and we are not liable for decisions made on it. Verify anything that matters against Medicare.gov’s Care Compare and with the facility.

Permitted use

The underlying government data is in the public domain and you may use it freely. Our arrangement, wording and derived counts are ours; you may quote and link to them with attribution. Do not scrape the site in a way that degrades it for others — the source files are published in bulk and linked from our sources page, which is a better route for any volume of data.

Liability

The site is provided “as is”. To the fullest extent the law allows, we exclude liability for indirect or consequential loss arising from its use.
